Required Forms for New Employees

The new hire packet includes forms required as part of the employee onboarding process. Please print, fill out, and return the relevant documents to human resources prior to receiving your first paycheck.

In addition to completing the documents contained in the new hire packet, all employees must provide human resources a copy of their:

  • Driver’s License

  • Auto Insurance Card

  • Social Security Card

Pay Dates and Policies

UMC Food Ministry employees are paid on a weekly basis.

ALL EMPLOYEES MUST USE DIRECT DEPOSIT.

UMC Food Ministry carries liability, workers compensation insurance, hired and unowned auto insurance, and Directors and Officers insurance. You may view a copy of our coverage certificate here.
